Aging Care Connections

Aging Care Connections is a leader in providing client-centered care to older adults in Chicago’s west suburban townships. Based in La Grange, IL, Aging Care Connections is a non-profit organization that serves over 8,000 older adults, caregivers and their families from 38 communities in the Lyons, Proviso, Riverside, Leyden and Norwood Park townships. The mission of Aging Care Connections is to enrich the quality of life in our community by providing older adults and their families with guidance and support that enhances the ability to age well. Since first opening its doors in 1971, Aging Care Connections has grown and developed to offer a comprehensive range of programs and services specifically designed to meet the changing needs of our community’s aging population. Professional staff provides clients with community-based services using a social work lens, links clients with other community organizations as needed, and works to build a culture of health in our service area. Our programs include: Adult Protective Services, Transitional Care, Aging Well Neighborhood, Benefits Assistance, Caregiver Support, Comprehensive Care Coordination, Congregate Meals, Specialized Diet Home Delivered Meals, and Information and Assistance.

Community Support Solutions

As a Family-Centered Support Agency, Community Support Solutions serves Care recipients and family with our policies, practices, and systems that are family-centered by using the individual and developmental approach. We believe when working with children, parents are the experts in their child’s life. It is our job to work together in the best interest of the child and the family. Working with adults as Care recipients we practice person-centered planning while recognizing that some adults with disabilities experience challenges in understanding and communicating their preferences and needs, and in those circumstances while keeping the client’s choice in mind, we work with family members who often play a critical role in ensuring that those preferences are honored and needs are met.

VIA Centers for Neurodevelopment is dedicated to helping people overcome the challenges of autism through innovative, evidence-based programs in education, outreach, and adult services. Our programs and services are built around the principles of Applied Behavior Analysis and employ professionals in the fields of behavior analysis, childhood special education, speech-language therapy, clinical psychology, and occupational and physical therapies.
